Say this

  1. Report every incident, however small. That includes near misses, damage and minor cuts.
  2. If someone is hurt, make the area safe first. Switch off the machine. Then give first aid or call for help.
  3. Do not tidy up the scene. Leave tools and materials where they are so we can see what happened.
  4. Tell me or the safety officer straight away. The company has legal deadlines for reporting to MOM.
  5. If you saw it happen, we will talk to you on your own. Tell us what you saw, not what you think we want to hear.
  6. We are looking for what failed in the system. Was the procedure wrong, the tool worn or the plan rushed? Blame does not stop the next one.
  7. When we fix something, we will tell you what changed. If the fix does not work on the ground, say so.

Ask the crew

  1. What counts as a near miss on this site? Give me an example from this week.
  2. Someone is hurt by a machine. What are the first two things you do?
  3. Why should you leave the scene untouched?
  4. Who do you report to if I am not here?
